We're hiring a Director of Social Services for a skilled nursing facility in Newark, NJ. This is a full-time, in-house leadership role for a Director of Social Services who holds a Master of Social Work (MSW) and has already run a social services department in a nursing home. If you've been a Director of Social Services in a skilled nursing facility (SNF) or long-term care (LTC) setting and you're ready for a building that values the work you do, we want to talk to you.

Prior nursing home experience is required. Our Director of Social Services owns the resident experience from admission through discharge: psychosocial assessments, care planning, family communication, discharge planning, and resident rights.

What You'll Do

• Lead the Social Services department, including supervising social services staff and coordinating with nursing, therapy, and administration

• Complete psychosocial assessments and the social services portions of the MDS (Minimum Data Set) and resident care plans

• Manage discharge planning and coordinate safe transitions to home or community settings

• Serve as the main point of contact for residents and families, and handle grievances and resident rights concerns

• Keep the department in compliance with New Jersey Department of Health and CMS (Centers for Medicare & Medicaid Services) social services requirements

Qualifications

• Master of Social Work (MSW) required

• Prior experience as a Director of Social Services in a skilled nursing facility (SNF), nursing home, or long-term care (LTC) setting is required

• LSW (Licensed Social Worker) or LCSW (Licensed Clinical Social Worker) in New Jersey preferred
