Job Description Job Responsibilities The post holder will be an experienced general practice pharmacist working within their professional boundaries, supported by GreenLight pharmacists and practice GPs. They will take on clinical pharmacy leadership roles, including: Conducting Structured Medication Reviews Managing chronic disease areas Medicines optimization Quality improvement initiatives Overseeing the quality and outcomes framework Leading, training, and coordinating less experienced pharmacy professionals Managing repeat prescribing systems Addressing acute prescription requests and medicines reconciliation Providing clinical medicines advice and addressing public and social care needs This role is crucial for improving quality of care and operational efficiencies, requiring motivation and passion for delivering excellent service in general practice. Primary Duties and Areas of Responsibility Patient Facing: Clinical Medication Reviews, care home medication reviews, domiciliary clinical medication review, medicines support. Telephone Support: Medicines support. Practice Staff Support: Providing medicine information. Hospital Admissions: Unplanned admissions management. Documents Management: Repeat prescribing and signposting. Risk Stratification: Service development and information management. Quality Improvement: Medicines quality improvement and safety. Required Skills and Attributes Essential Qualifications Education: Masters degree in pharmacy (MPharm) or equivalent.
