ViaQuest Healthcare Central Full-Time | $50,000–$53,000 per year | On-Call for Incident Reporting At ViaQuest Healthcare Central, we provide high-quality, person-centered care to individuals with developmental disabilities in our Intermediate Care Facilities (ICF/IID). Key Responsibilities Ensure compliance with state and federal ICF/IID regulations Develop, implement, and monitor Individual Plans (IPs) Conduct annual, quarterly, and special team meetings Complete Comprehensive Functional Assessments and documentation Maintain accurate and up-to-date records and floor books Monitor services and support implementation in day programming Coordinate appointments and community-based services Oversee medication and medical supply processes, as appropriate Participate in on-call incident reporting rotation Qualifications Bachelor’s degree in Social Services or related field Minimum 1 year of experience in developmental disabilities preferred Strong communication, customer service, and organizational skills Ability to manage multiple priorities and meet deadlines Supervisory or leadership experience preferred Why Join ViaQuest?
