Job Responsibilities

• Evaluate patients in both inpatient and outpatient environments, tailoring unique treatment plans for each individual
• Implement physical therapy interventions aimed at enhancing mobility, strength, balance, and function
• Collaborate closely with nurses, physicians, case managers, and rehab professionals to maximize patient outcomes
• Educate patients and their families regarding therapeutic goals, effective discharge planning, and home exercise routines
• Maintain thorough documentation of evaluations, treatments, and progress in compliance with facility and regulatory requirements
• Engage in care team meetings and interdisciplinary discussions to ensure cohesive patient care

Education

• Doctorate in Physical Therapy (DPT) or a relevant degree from an accredited institution

Certifications

• Current state licensure as a Physical Therapist
• CPR certification preferred
• Specialized certifications (e.g., in neurology, geriatrics) can be advantageous but are not mandatory

• Prior experience is strongly preferred
