You'll work alongside a dedicated healthcare team while expanding your clinical expertise in vascular care management.Local travel is an essential part of this role, with approximately 75% travel required to support patient care needs and collaborate with healthcare partners throughout the community. This role will require you to see patients in their care facilities, homes or in our satellite offices. You will be partnered with an Ultrasound Tech and will work to serve our patients as a team.What You'll DoPerform comprehensive patient assessments, physical examinations, and medical history reviews.Order, interpret, and evaluate diagnostic testing, including laboratory studies and imaging.Diagnose and treat acute and chronic vascular conditions.Develop, implement, and manage individualized treatment plans.Prescribe medications and monitor treatment effectiveness.Educate patients and caregivers on health conditions, preventive care, and treatment plans.Collaborate with referring providers and interdisciplinary care teams to ensure seamless patient care.Maintain accurate and timely clinical documentation.Monitor patient progress and adjust care plans as needed to achieve optimal health outcomes.What We're Looking ForExperience practicing as an Advanced Practice Nurse in clinical settings such as primary care, hospitals, outpatient clinics, emergency medicine, or related healthcare environments.
