Summary In collaboration with other providers, identify, engage, support, and retain individuals with co-occurring substance use and mental health disorders who have experienced issues with commitment or follow through in the past, and/or not benefited from traditional behavioral health treatment services. The role focuses on supporting individuals’ recovery and overall well‑being through engagement in treatment or rehabilitative services and activities to functionally prepare for, find, obtain, and maintain competitive employment. ESSENTIAL DUTIES AND RESPONSIBILITIES Work in collaboration with partner provider agency personnel to coordinate and operate services at the Taking Initiative Center (TIC) located in New Haven, CT. Coordinate intake process for new referrals to include review of any referral materials; conduct a comprehensive, in‑depth interview with the individual and identify and engage individuals who have experienced issues with commitment or follow through in the past, and/or not benefited from traditional behavioral health treatment services. Conduct comprehensive assessments of psychosocial, vocational, and rehabilitative needs and provide individual counseling and connect individuals to appropriate training and treatment resources. Make individual referrals to other community agencies for care and services not provided by the Agency, i.e. respite, drug rehab, soup kitchens, housing/homeless shelters, etc. Assist individuals in preparing for employment through job readiness training, functional skills development, and vocational counseling as well as supporting individuals in job search, application, and interview processes. Offer ongoing support to help individuals maintain employment and address workplace challenges. Recognize the physical and behavioral indicators of probable drug or alcohol abuse and intervene for individuals in crises, emergency or problem situations; counsel or make referrals to appropriate medical, psychiatric or other treatment personnel as deemed appropriate and to ensure the safety of the individual and of others. Be knowledgeable of Alcoholics Anonymous (AA) and Narcotics Anonymous (NA) meeting schedules and support groups. Facilitate groups and/or support the participation of individuals in such groups. Encourage individuals receiving services to maximize their input regarding services/supports that may be provided by supporting self‑advocacy activities. Provide a daily activity schedule for individuals. Work collaboratively with other providers and community organizations for program development and individual service planning, and participate in regular (at least quarterly) meetings with partner agencies to ensure coordinated care. Provide required communication and documentation to meet agency, accrediting body, state, federal and other pertinent standards and guidelines. Maintain accurate and timely records of individual progress, service delivery, and outcomes. Submit required data and reports to the Department's system by the 15th of each month. Attend lectures, workshops and seminars to enhance skills in the areas including, but not limited to, substance abuse, job training and employment. EDUCATION And/or EXPERIENCE Bachelor's degree from four‑year college or university in a related field; three to five years related experience; or equivalent combination of education and experience. Mathematical Skills Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio and percent. REASONING ABILITY Ability to solve practical problems and deal with a variety of variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written or oral form. Ability to assist workers with appropriate social behavior to facilitate integration and inclusion. Ability to act as an advocate for workers. Other Skills And Abilities Familiarity with infectious diseases, i.e., Hepatitis B, HIV/AIDS, Tuberculosis, etc., and appropriate infection control/universal precautions. Ability to obtain CPR and First Aid certifications. CERTIFICATES, LICENSES, REGISTRATIONS Valid Connecticut driver's license. Job responsibilities may require use of personal vehicle.
