Physical Therapist - Home Health

Home Health Physical Therapist (PT) – Flexible Schedule

We are seeking a licensed Physical Therapist (PT) to provide high-quality, patient-centered care in a home health setting. In this role, you will evaluate, treat, and support patients in improving mobility, reducing pain, and restoring function—all within the comfort of their homes.

Key Responsibilities

• Evaluate patient function and develop individualized treatment plans in collaboration with the physician
• Perform diagnostic and functional assessments (muscle, nerve, joint, and mobility)
• Provide therapeutic interventions to relieve pain and improve mobility and independence
• Educate patients and families on exercises, treatment plans, and use of assistive devices (canes, walkers, wheelchairs, etc.)
• Communicate regularly with the Nurse/Case Manager and interdisciplinary team
• Document patient progress, treatment responses, and changes in condition in a timely manner
• Participate in development and updates of the Plan of Care
• Coordinate outpatient services when needed

Requirements

• Graduate of an accredited Physical Therapy program
• Current Physical Therapist (PT) license in the State of Michigan
• Minimum of one (1) year of experience preferred (home health or clinical setting)
• Valid driver's license, reliable transportation, and auto insurance
• Ability to meet physical demands including lifting, transferring, and assisting patients
• CPR certification preferred
