POSITION SUMMARY - RN Case Manager


POSITION DUTIES - Coordinates and oversees patients' individualized rehabilitation plans, collaborating with physicians, therapists, patients, and families to ensure appropriate care, discharge planning, resource utilization, and achievement of optimal functional outcomes.


MINIMUM REQUIRED QUALIFICATIONS -

• OK/compact RN license
• BLS
• 1+ year recent acute case management experience in a hospital setting

PREFERRED QUALIFICATIONS - Acute inpatient rehab and/or med/surg experience


LENGTH OF ASSIGNMENT - 13 weeks


SHIFT / HOURS PER WEEK - 8a-4:30p. Weekends on request.
