Hospice RN Case Manager - Coordinated Home Care
Affinity Hospice Management LLC
Affinity Hospice Management LLC is seeking a Registered Nurse to plan, organize and direct hospice care in home settings. The RN will perform comprehensive patient assessments, develop and revise care plans, administer medications, educate patients and families, and act as Case Manager as needed.
