About the Position

Practice beyond the walls of a traditional clinic as a full-time Mobile Nurse Practitioner serving patients throughout San Gabriel Valley and East Los Angeles, California. In this role, you will bring high-quality, relationship-based care directly to patients through home visits, community-based care, and virtual appointments while working within a patient-centered, value-based care organization focused on improving outcomes and supporting long-term health.

Our care model is focused on quality outcomes, preventive care, proactive interventions, and coordinated patient support rather than high visit volume. You will enjoy a high level of clinical autonomy while working alongside an interdisciplinary team dedicated to helping patients achieve better health outcomes.

Position Highlights

• Full-Time Schedule: Monday–Friday | 8:00 AM–5:00 PM
• Competitive Base Salary: $130,159-$195,238
• $10,000 Sign-On Bonus

Qualifications

• Active and unrestricted California Nurse Practitioner license
• Active DEA license
• Current BLS certification
• Strong clinical assessment and diagnostic skills
• Ability to practice independently while collaborating effectively with an interdisciplinary care team
• Commitment to patient-centered, preventive, and value-based care
• Excellent communication and relationship-building skills
• Comfortable providing care in home, community, and virtual settings
