Our Hospice Visiting Nurse (RN) utilize their extensive education and training to provide personalized care to our patients in a rewarding environment that promotes autonomy.Job DescriptionSupport our patients, families, and loved ones, as they go through their end of life journeyPerform a comprehensive assessment to establish goals for symptom management and end of life care with effective case management strategiesBe a member of an active interdisciplinary team that addresses patients' needs holisticallyDevelop and maintain a therapeutic relationship by utilizing effective communication with patients and their familiesUtilize state-of-the-art technology to document skilled services providedQualificationsEducation: Graduate of an Associate's of diploma program in nursing required. Graduate of NLN approved BSN program preferred.Experience: One year nursing experience OR one year nursing experience in a specialty required. Medical/surgery or home care experience preferred.Licensure, Certification, Registration: License to practice in the State of Connecticut required.Knowledge, Skills and Ability Requirements: Computer skills preferredWe take great care of careers.
