This role collaborates closely with the Interdisciplinary Team (IDT) to ensure holistic, culturally sensitive care aligned with hospice philosophy and regulatory standards.Key ResponsibilitiesAssess spiritual needs and provide supportive spiritual care to patients and familiesDevelop, implement, and document individualized spiritual care plansCollaborate with the Interdisciplinary Team to coordinate holistic careParticipate in IDT meetings and care plan reviewsProvide consultation, education, and support to staff regarding spiritual careCoordinate with clergy, faith leaders, and community spiritual resources as requestedConduct pre-bereavement and bereavement assessments, visits, and follow-upsFacilitate or conduct funeral and memorial services for patients, families, facilities, and staff as requestedNetwork with faith communities to educate on hospice philosophySupervise spiritual care volunteers as assignedManage caseload, scheduling, and travel efficientlyRespond flexibly to unscheduled events, admissions, or emergenciesMaintain compliance with all local, state, federal, and Medicare regulationsParticipate in evening, weekend, holiday, and on-call rotations as requiredMaintain professional, ethical conduct and regular attendanceFoster a positive, respectful, and engaging work environmentQualificationsMinimum age of 18Graduate of an accredited seminary or school of theology (B.D., M.Div., or equivalent), or degree in counselingExperience working with death and dyingAbility to work effectively within an interdisciplinary teamComfort working in an interfaith, multicultural environmentKnowledge of and commitment to hospice philosophyHospice or home health experience preferredStrong communication, planning, and organizational skillsSelf-directed with the ability to work independentlyValid driver's license, reliable insured transportation, and acceptable driving recordAbility to meet physical and health requirements, including TB screening and physical exam per state and company policySatisfactory professional referencesExperience PreferredHospice Care: 1 yearOur PromiseAt Vitality Hospice & Palliative Care, your work makes a difference—every day.
