Hours: 

• M-F, 9:00am-5:00pm
• Weekend rotation (ok to split, 2 Saturdays or 2 Sundays)

• Provide hands-on care to patients in their home

• Provide initial visits and revisits to patients

• Woundcare, medication administration

• Performs patient comprehensive assessments, as assigned, and collaborates with patient multi-disciplinary care team

• Completes documentation timely, accurately, and at the point of care, according to industry standards, conventions and guidelines, including OASIS assessments, SOC/Admissions, Recertifications, ABN's, subsequent visits, physician orders, care coordination

• RN, 1 year Post Acute Care OR Acute Hospital OR CHHA
• Valid Driver's License and car required
