Rehabilitation Specialist

Rehabilitation Specialist Community Bridges, Inc. (CBI) is an integrated behavioral healthcare agency offering residential, outpatient, inpatient, patient‑centered medical homes, medication‑assisted treatment and crisis services to individuals experiencing crisis, opioid use disorder, homelessness, and mental illness throughout Arizona. Job Summary The Rehabilitation Specialist educates team members on community activities and facilitates member connection to employment and rehabilitation services. They assess, coordinate, link, monitor and provide direct supportive and skills‑based services in a community setting for members. This includes utilizing the Vocational Activity Profile or a similar tool to assist members in identifying interests and connecting to supportive services, as well as participating in the development of the member's individualized service plan (ISP) and initiating and coordinating services related to ISP goals. The specialist is familiar with the referral process and completes referrals in a timely manner. Education and Experience Required Bachelor’s degree in a field related to behavioral health or a combination of education, training, and/or experience meeting A.A.C. R9-10-101.33 and CBI Clinical Policies and Procedure standards for BHT. One year of experience working with the behavioral health population, with additional experience in multiple settings and levels of care, including both mental health and substance abuse. Ability to pass a criminal background check for jail visitation/clearance.
