Category: Pharmacy Posting #: 492372 Employee Type: Varies Position Summary We are looking for a dedicated inpatient clinical pharmacist to join our team at the Thomas Campus, focusing on supporting the medication needs of our pediatric patients. This 8‑hour evening shift position offers a unique opportunity to make a meaningful impact during non‑traditional hours. Incentives include an evening shift differential, relocation assistance for qualified candidates, and a starting bonus up to $10,000. This position oversees the medication management process and provides comprehensive pharmacy services to patients from neonate to young adulthood while performing the duties of a registered pharmacist as defined by the Arizona State Board of Pharmacy and Phoenix Children’s. It ensures compliance with federal, state, and local laws and regulations as well as proper storage and documentation. It receives supervision and leadership oversight from Inpatient Pharmacy leaders. Position Duties Review, manage, and verify medication orders for therapeutic appropriateness based on age, weight, height, laboratory values, diagnosis, and other clinical information. Screen for and manage clinical alerts such as allergies, interactions, therapeutic duplications, and adverse drug reactions. Problem‑solve issues with medication orders and consult prescribers for alternative therapies. Comply with Pharmacy and Therapeutics Committee directives related to formulary management. Document clinical interventions to meet cost‑containment and evidence‑based medication therapy goals. Prepare and dispense sterile and non‑sterile medications in compliance with policies, procedures, and laws. Utilize clinical pharmacy skills to assess and formulate medication therapy recommendations for medical staff. Participate in decentralized pharmacist‑assigned duties when applicable. Assist in developing guidelines, policies, and monitoring strategies for medication use. Provide ongoing education and training to pharmacists, residents, interns, technicians, and students. Supervise technical, intern, and support staff to ensure accuracy in medication preparation and dispensing. Act as liaison between pharmacy and medical and nursing staff on medication management across units. Participate in hospital and departmental operational and quality improvement initiatives. Communicate problems, unique patient needs, and other essential information to incoming staff. Initiate, process, and complete incident, adverse drug reaction, and controlled substance documentation reports. Engage in drug‑use evaluation, quality assurance, and performance improvement activities. Maintain investigational drug study records and documentation as required. Provide evidence‑based medication education and information to patients, caregivers, and staff. Counsel patients and caregivers for outpatient prescriptions. Provide targeted drug information and home medication screening. Perform miscellaneous job‑related duties as requested. Position Qualifications Education: Bachelor’s Degree (BS) or Doctorate Degree (Pharm.D.) in Pharmacy. Must have graduated from an accredited pharmacy school or be graduating within the next 6 months. (Required) Education: Completion of a post‑graduate year 1 (PGY‑1) pharmacy residency program accredited by the American Society of Health‑System Pharmacists (ASHP). (Preferred) Experience: Proficiency with information systems and automated equipment. (Required) Experience: One year of experience in a hospital pharmacy practice or equivalent. (Preferred) Experience: One year of experience with pediatric patient populations or equivalent. (Preferred) Special Skills: Proficiency with computer software, including MS Office Suite or equivalent. (Required) Special Skills: Demonstrated ability to problem‑solve and collaborate with team members to support a cohesive work environment. (Required) Additional Requirements: Current licensure as a Pharmacist or eligibility by reciprocity or examination by the Arizona State Board of Pharmacy. (Required) Additional Requirements: Current BLS and PALS or ACLS for Healthcare Provider certification from the American Heart Association.
