RN Case Manager: Care Coordination & Discharge Planning

TriStar Health in Bowling Green, KY is seeking an RN Case Manager to coordinate patient care within our Healthcare & Medical Services department. In this full-time role, you will perform comprehensive assessments, develop and manage discharge plans, and guide patients and families through the continuum of care. You’ll collaborate closely with physicians and interdisciplinary teams, ensuring appropriate utilization of services and smooth care transitions. #J-18808-Ljbffr
