Overview

Responsibilities
- Medical necessity determination and utilization review
- Discharge planning and care coordination
- Document initial screenings and discharge plans in Epic
- Communicate changes in plan of care to the care team; maintain community resource knowledge
- Use LOS data to anticipate needs and facilitate transitions of care
- Manage continued-stay reviews, appeals/denials as needed

Requirements
- Active RN license for Idaho; in-hand at submittal; state or compact license accepted
- BLS certification
- 2 years of case management experience preferred; prior charge experience preferred
- Epic charting experience required
- Experience with adults, pediatrics, and geriatrics
- Onboarding: complete core items and required labs within 48 hours of contract signing
