Nurse Practitioner Or Physician AssistantWe are seeking an experienced Nurse Practitioner or Physician Assistant for a locums assignment to convert to Perm (with in 3-6 months) at a primary care practice serving geriatric patients in a value-based care model. This is a hybrid role with blend of coverage in outpatient clinics as well as home-based patient care.Compensation: $75 – $95 per hour per dayPosition Details:Schedule: Full Time Hours GuaranteedMonday – Friday - 5 Days Per Week8:30 Start TimePatient Load: LOW DAILY CENSUSHome Care: 3–5 patients per dayClinic: Small census of 7–10 patients per daySetting: Hybrid (Outpatient clinic + home-based care), value-based primary care for elderly patientsResponsibilities:Deliver comprehensive primary care services, including preventive care, chronic disease management, and acute care to Geriatric patientsDiagnose, treat, prescribe medications, and develop individualized treatment plansOrder and interpret diagnostic tests (labs, imaging, etc.)Document accurately in the EHR systemProvide patient education and promote wellness initiativesCollaborate with physicians, nurses, and support staff to ensure coordinated, high-quality careRefer patients to specialists and manage follow-upsEnsure compliance with state, federal, and professional regulationsRequirements:Active NP or PA license in NYMinimum 2 years of primary care experienceNational Board CertificationExpedited CredentialingMedicaid and Medicare Provider Id required
