Clinical Case Manager – Treatment Foster Care At Nexus–Woodbourne, we provide children and youth (ages 0–21) with a mental health diagnosis involving severe emotional and behavioral problems safe, specially equipped foster homes that have the skills and resources to meet their complex needs. Position Summary The Clinical Case Manager provides assessment and treatment services to individuals, families and groups and coordinates ongoing therapeutic services and community support necessary to ensure clients receive stabilization care and resources necessary for their daily living activities. Primary Responsibilities Assists clients in linking to services, identifying barriers, and connecting with supports prior to discharge Provides consultation and support to families, youth, and adults in need of services Works closely with teams to create stabilization and discharge plans, develop treatment plans, and utilize a variety of therapeutic approaches Screens direct referrals, assists in triaging individuals, and provides clinical assessments and mental health therapy Provides assistance to families regarding available resources Serves as a resource for crisis intervention Communicates with families, legal systems, schools, mental health providers, probation, and social service agencies Treatment Team Collaboration & Service Delivery Provides timely and accurate communication to supervisory and management staff regarding significant events Maintains and completes required paperwork and documentation for governing/accrediting agencies; maintains current files on all individuals enrolled in services Produces informal and formal reports as requested by outside agencies, parents, referral agencies, courts, etc. Facilitates discharge planning meetings and develops discharge plans that meet individual needs Completes the aftercare process of discharge planning, follows up on connections to outside services, and conducts satisfaction services for providers and individuals receiving services Outside Organization Collaboration & Support Builds relationships with adults, families, youth, and outside organizations and advocates for needs Works effectively with outside organizations to implement plans for adults and children; communicates with families, legal systems, schools, mental health providers, probation, and social services Maintains database of community resources and contacts for outside providers and organizations Creates aftercare plans and follows up Participates in staff and supervision meetings and required in‑service meetings Policy, Procedure, Compliance & Training Exhibits professional, respectful, and effective communication when interfacing with clients, coworkers, families, and external contacts Demonstrates empathy and maintains appropriate boundaries Exhibits high ethical conduct, integrity, confidentiality, and adherence to the Social Work Code of Ethics Shows appreciation for people from diverse backgrounds; exhibits understanding and acceptance of differences in gender, race, age, culture, ethnicity, class, religion and disability Maintains a flexible schedule to meet changing work needs and client demands Handles emergency or crisis situations with sound judgment Utilizes organizational skills and tools to proactively manage multiple client needs and demands Represents the organization positively within the community ICARE Values & Behavioral Competencies Innovation: Leading the way and implementing creative, cutting‑edge ideas and approaches Compassion: Listening, honoring differences, and showing respect, kindness, empathy, care, and concern Agility: Exhibiting flexibility and adapting quickly Responsiveness: Being quick, positive, and accurate Excellence: Demonstrating quality results that surpass ordinary standards Requirements Education and Licensure (Required) Master’s degree in social work or a clinically related field in Human Services Minimum LMSW licensure level Valid driver’s license; meet state regulating agency driving requirements and maintain auto insurance and pass corporate insurance eligibility standards All client‑facing employees must successfully complete a Safe Crisis Management behavioral intervention method exam and training Preferred Education & Experience LCSW, LCSW‑C licensure Previous experience in social work or treatment foster care Salary Description $62,500 – $80,000 Equal Employment Opportunity Statement At Nexus Family Healing, our voices and actions are focused on recognizing, affirming and respecting people of every race, ethnic background, socio‑economic status, sexual orientation, gender expression and faith. #J-18808-Ljbffr Nexuspath
