Licensed Social Worker (LSW) – Full-Time
CCH Healthcare | Cincinnati, OH Area
CCH Healthcare is seeking a compassionate, driven, and experienced Licensed Social Worker (LSW) to join our skilled nursing and long-term care team in the Cincinnati, OH area. This role is ideal for a social worker who is passionate about resident advocacy, psychosocial support, and improving quality outcomes in a long-term care setting.

Assess and address residents’ psychosocial, emotional, and mental health needs
Complete admissions, care plans, and discharge planning documentation
Collaborate with nursing, therapy, and leadership teams to support resident outcomes
Advocate for resident rights, dignity, and individualized care planning
Provide counseling and support to residents and families
Coordinate and maintain community resources and services
Ensure timely, accurate documentation in compliance with regulatory standards
Support implementation of policies related to social services and resident care
Active LSW license in the State of Ohio (REQUIRED)
Minimum 2 years of social work experience in long-term care or skilled nursing
Strong communication, organizational, and interpersonal skills
Ability to work effectively in a fast-paced, team-oriented environment
Passion for working with geriatric populations and advocating for resident well-being
