Prava Therapy is seeking a full-time Occupational Therapist to join our team within a vibrant Independent Living community in Apex, North Carolina. You'll get to know your residents, their routines, and the activities that matter to them most. YOUR ROLE As an Occupational Therapist, you'll provide skilled clinical care while becoming a trusted partner within the community. You'll: Perform comprehensive evaluations and develop individualized plans of care. Provide skilled treatment addressing ADLs, IADLs, functional mobility, upper extremity function, adaptive strategies, cognitive-functional performance, and daily living independence. Connect therapy interventions to meaningful, resident-centered goals. Build strong relationships with residents and understand what matters most to them. Identify changes in function and opportunities for early intervention. Educate residents, families, caregivers, and community team members on home/apartment safety, adaptive equipment, energy conservation, and independence. Collaborate with physicians, community leadership, wellness professionals, and other members of the care team. Monitor progress and modify treatment plans based on clinical presentation and functional outcomes. Maintain timely, accurate, and compliant documentation. Participate in care coordination and discharge planning as appropriate. Contribute to a culture of clinical excellence, compassion, professionalism, and continuous learning.
