The role requires seasoned case management experience in an adult acute hospital setting. The facility is a state-of-the-art, inpatient acute rehabilitation hospital. It focuses on treating and recovering individuals from severe injuries or illnesses. Applicants should have experience working with diverse patient populations. Common diagnoses include stroke, medical debilitation, and post-surgical conditions. The position involves participating in team conferences and supporting family discussions. Strong organizational and time management skills are essential for this role. Candidates should have basic computer skills, including knowledge of Excel, Word, and Outlook. Effective oral and written communication skills in English are necessary. Ability to interpret information and work independently is required.
