Registered Nurse - Complex Pediatric Case Manager

Schedule

This position requires a full-time commitment with shifts from 8:00 AM to 5:00 PM, Monday through Friday. Some weekend work and after-hours may be necessary to meet patient needs.

Job Description

The Registered Nurse in this role will provide expertise and leadership in managing complex pediatric patients, handling up to ten cases a day. This position is crucial for enhancing patient care through effective resource management and continuity of care. The Case Manager will be involved in comprehensive assessments, planning, interventions, and evaluations regarding patients’ appropriate levels of care.

Responsibilities

• Monitor length of stay (LOS) and implement safe discharge plans.
• Assess assigned patient populations for medical necessity and appropriateness of care.
• Collaborate with social workers and healthcare teams to ensure timely discharge planning.
• Document delays in care and readmissions to identify trends and improve interventions.
• Participate in interdisciplinary patient care rounds to develop effective discharge strategies.
• Maintain compliance with regulatory agencies and attend required meetings.
• Adapt care methods to meet the unique needs of diverse patient populations.
• Act as a change agent within the organization, gathering support for new initiatives.

Required Experience / Certifications / Licensure

• Graduated from an accredited school of nursing; BSN preferred.
• Current RN license in the State of Texas required.
• A minimum of 5 years of clinical practice experience in pediatrics.
• Experience in case management or resource management preferred.
• Familiarity with Milliman (MCG) guidelines and Interqual criteria is highly preferred.
• Certification in Case Management is a plus.
