PRN Hospice RN Case Manager - Home-Based Care

St. Mary’s Home Care and Hospice in Bucks County, PA is seeking a contingent/PRN Hospice RN Case Manager. The RN will assess, plan, and manage care for hospice patients at home, collaborating with physicians and an interdisciplinary team. Requirements include PA RN licensure and at least 2 years of nursing experience in acute care, home care, or ER, with home care experience preferred.
