Job Description
Title: RN Case Manager - Home Health

Schedule: 8:00 AM - 4:30 PM

Assignment Details:
- Contract Length: 12 weeks
- Guaranteed Hours: 40

Requirements:
- 2+ years of home health experience
- Case management experience
- OASIS experience
- Wound care experience preferred

Roles & Responsibilities:
- Conduct comprehensive patient assessments to identify healthcare needs.
- Develop, implement, and coordinate individualized patient care plans.
- Monitor patient progress and adjust care plans as necessary.
- Ensure compliance with all regulatory requirements related to case management.
- Maintain accurate and timely documentation of patient care and progress.
- Collaborate with healthcare teams to optimize patient outcomes.
