Provide skilled physical therapy services. Educate patients and caregivers on exercises, safety, and mobility. Complete OASIS and clinical documentation in a timely manner. Collaborate with physicians, nurses, and other healthcare professionals. Monitor patient progress and adjust treatment plans as needed. Promote safe, high-quality patient care and excellent outcomes. Qualifications Graduate of an accredited Physical Therapy program. Current Michigan Physical Therapist license. Current CPR certification. Minimum of 2 years of PT experience preferred.
