Job Description

Job Summary:
Provides clinical evaluation, treatment, and rehabilitation services to patients with physical functioning disorders and wound care needs. Develops and implements individualized care plans following regulatory and clinical standards to improve patient outcomes.

Responsibilities:

• Screen, assess, and evaluate patients to determine their physical therapy needs and develop appropriate treatment plans.
• Administer manual techniques, therapeutic exercises, modalities, wound care, and use of supportive devices as part of the rehabilitation process.
• Monitor patient progress, document clinical interventions, and modify treatments based on ongoing assessments and outcomes.
• Provide education, counseling, and support to patients, families, caregivers, and healthcare team members regarding treatment plans and progress.
• Ensure all documentation, including discharge summaries and treatment records, complies with regulatory and clinical requirements.
• Supervise and support physical therapist assistants, rehabilitation aides, and students, ensuring adherence to professional standards.

Qualifications:

• Possess at least 1 year of outpatient or rehabilitation experience in physical therapy.
• Hold an active MA State Physical Therapy license.
• Demonstrate proficiency in utilizing standardized assessments and evidence-based practices.
• Have experience in long-term care or rehabilitation settings.
• Ability to effectively communicate and collaborate with multidisciplinary teams.
• New graduates with Level II experience are welcome to apply.
