Key Responsibilities of the Physical Therapist

• Perform physical therapy evaluation and treatment of patients based upon physician orders/referrals
• Complete comprehensive evaluations using appropriate tools and procedures
• Review patient records for current diagnosis, past history, and precautions
• Establish measurable goals and develop a treatment plan
• Regularly re-assess treatment plans and patient progress
• Communicate patient needs and progress effectively
• Consult with medical providers regarding patient care
• Recommend and assist in securing adaptive equipment for patients
• Maintain and submit documentation as required
• Supervise Physical Therapist Assistants and Aides in patient-related activities
• Participate in staff development and continuing education

Requirements of the Physical Therapist

• Current Physical Therapist license
• Experience in home health required
• HCHB experience preferred but not required
