What You Must Have: Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing Current State Registered Nurse License Current Driver's License, reliable transportation, and provide ongoing valid and current auto liability insurance One to three years of experience as an RN in a clinical care setting, hospice or home health‑preferred. Full Time Employee Benefits: Competitive Pay Paid Time Off Paid holidays 401k with up to 4% employer matching Company car for qualifying individuals What You Will Do: Demonstrate autonomy, organization, assertiveness, flexibility and cooperation in performing job responsibilities. Review clinical referral information and respond to requests and inquiries as appropriate. Collaborate with Patient Care Manager, attending physician and hospice physician regarding the eligibility and appropriateness of client for hospice services. Conduct initial meeting with the patient and family over hospice service and philosophy and obtain consents for care. Provide the initial comprehensive assessment of immediate physical, psychological, social and spiritual needs of the hospice patient and family and initiate appropriate interventions. Develop an initial care plan that establishes immediate goals and interventions, based on nursing assessment and collaborate with the attending HMD and core team. Maintain regular communication with the Intake Coordinator, Patient Care Managers, RN Case Manager and core team to review patients and help with managing needs (this may include unusual or potentially problematic patient/family issues). Timely submission of all documentation. Performance of other duties as required. Ensure overall compliance with local, state and federal laws, Medicare regulations, and established personnel policies and procedures.
