Description

American Traveler is hiring an RN Case Manager for an acute care hospital setting requiring 3+ years of RN experience and a BSN.

Details

• Hospital Care Management unit within an acute care hospital
• Day shift schedule: 5 days/week, 8:00a-4:30p (40 hrs/week)
• Uses Epic charting system
• Coordinates care and discharge planning for patients identified as needing case management services

Requirements

• Active RN license (Montana or compact)
• BSN required
• BLS certification (AHA) that does not expire prior to start date
• Minimum 3 years of RN experience in an acute care setting required
• Hospital care management experience required
• 1 year of experience in case management specialty required
• Knowledge of medical terminology
• Proficiency with desktop software including PowerPoint and Excel for data entry
• ACM or CCM certification preferred but not required
• 5 years of nursing experience preferred

Additional Information

• Collects in-depth patient assessment data to identify needs and develop individualized care plans
• Collaborates with physicians and multidisciplinary care teams to coordinate services and discharge planning
• Identifies strategies to reduce length of stay and ensure appropriate utilization of resources
• Attends and participates in patient care rounds and case conferences
• Advocates for patient rights and monitors outcomes through variance tracking and data collection
• Educates physicians and nursing units on case management and discharge planning processes
• Returning staff must be separated from any Intermountain facility for at least one year before being eligible for a travel assignment
