Competitive Compensation Flexible Scheduling Supportive Clinical and Administrative Team Meaningful OneonOne Patient Care Clinical Autonomy Opportunities for Growth and Professional Development Positive and Collaborative Work Environment Responsibilities Perform comprehensive occupational therapy evaluations in the home setting Develop individualized treatment plans based on patient goals and clinical needs Provide skilled occupational therapy services in patients' homes Improve patients' ability to safely perform Activities of Daily Living (ADLs) and Instrumental Activities of Daily Living (IADLs) Recommend adaptive equipment and home modifications to enhance safety and independence Educate patients and caregivers on therapeutic techniques, fall prevention, energy conservation, and home safety Collaborate with physicians, nurses, physical therapists, speech therapists, and other healthcare professionals Monitor patient progress and modify treatment plans as appropriate Maintain accurate and timely clinical documentation Complete all patient notes and communications within 24 hours Ensure compliance with agency policies and applicable regulations Travel to patient homes throughout assigned coverage areas Qualifications Active Pennsylvania Occupational Therapist (OT) License Graduate of an accredited Occupational Therapy program Home Health experience preferred Strong clinical assessment and treatment skills Excellent communication and interpersonal skills Strong computer and documentation skills Reliable transportation and a valid driver's license Ability to work independently and effectively manage a flexible schedule Commitment to delivering exceptional patientcentered care Coverage Areas Northeast Philadelphia Delaware County Chester County Montgomery County Bucks County Lehigh County Make a Difference Every Day As a Home Health Occupational Therapist, you will work directly with patients in their own environment, helping them maximize independence, improve safety, and enhance their overall quality of life.
