- Conduct thorough evaluations of patients in both inpatient and outpatient environments, crafting tailored treatment plans for each individual
- Implement physical therapy techniques concentrated on enhancing mobility, strength, stability, and overall functionality
- Work collaboratively with nurses, physicians, case managers, and other rehabilitation professionals to maximize patient outcomes
- Instruct patients and their families on therapeutic objectives, discharge procedures, and at-home exercise routines
- Document assessments, treatments, and progress as per facility and regulatory standards
- Engage in team rounds and interdisciplinary conferences to ensure cohesive care delivery
- Doctor of Physical Therapy (DPT) or equivalent credential from an accredited institution
- Active state licensure as a Physical Therapist
- CPR certification is preferred
- Additional specialized certifications (such as in neurology or geriatrics) are advantageous but not mandatory
- Prior clinical experience is strongly encouraged
- Adept at treating patients with intricate medical conditions and mobility challenges
- Exceptional communication and teamwork abilities
- Proficient with digital charting and clinical documentation systems
