Role Description The Registered Nurse at Apex Adult Foster Care is a full-time, on-site position based in Malden, MA. The nurse is responsible for assessing residents’ health status, developing and updating care plans, administering medications, and performing clinical procedures in accordance with regulatory standards and organizational policies. Day-to-day duties include monitoring vital signs, documenting clinical observations, coordinating with physicians and other healthcare providers, and educating residents and caregivers on health management, medications, and preventive care. The role also involves supporting direct care staff with clinical guidance, participating in interdisciplinary care conferences, responding to changes in residents’ conditions, and contributing to quality improvement and compliance activities. The Registered Nurse helps maintain a safe environment, promotes residents’ autonomy and comfort, and upholds professional and ethical standards of practice. Qualifications Current, unrestricted Registered Nurse (RN) license in the state of Massachusetts; CPR/BLS certification and familiarity with adult foster care or long-term care settings are highly beneficial. Demonstrated clinical skills in adult and geriatric nursing, including medication administration, health assessment, care planning, and monitoring of chronic conditions. Strong documentation and organizational abilities, with experience using electronic health records and maintaining accurate, timely clinical notes and care plans. Excellent communication and interpersonal skills to collaborate effectively with residents, families, physicians, and multidisciplinary care teams. Ability to work independently on-site, exercise sound clinical judgment, manage multiple priorities, and respond calmly to urgent or emergent situations.
