Primary Care Nurse Practitioner Float
The Advanced Practice Provider (APP) is a Nurse Practitioner (NP) or Physician Assistant (PA) who is part of a multidisciplinary team that provides comprehensive medical care to patients. This position is responsible for providing float coverage to outpatient community health centers throughout Ohio. When not providing float coverage this position will assist with timely management of in basket functions, such as patient messages and medication refill requests, for medical providers across the five Equitas Health Centers. The APP works autonomously and in collaboration with a variety of staff to diagnose and manage patients' health care. Participation in the development, expansion, and maintenance of the Equitas Health Medical Center program is expected.
Salary: $105,600-$132,000 with a $7,500 sign on bonus. This role would float between our medical center offices located in: Columbus, Dayton, Cincinnati, Toledo, and Akron.
Benefits: PTO, Vision, Dental, Health, 401k, Sick time. Mileage reimbursement, $5,000 travel stipend to reimburse the provider for any additional expenses they may incur traveling to one of the five clinics. Up to $5,000 relocation assistance.
Major areas of responsibilities include providing short and extended clinical coverage that includes direct patient care, remotely managing Epic In Basket, providing direct medical services to patients, prescribing or recommending pharmacologic and non-pharmacologic therapies, facilitating direct patient admissions into hospitals when needed, playing a key role on multidisciplinary team to develop coordinated treatment plans, using primary care clinic protocols and procedures in providing services, consulting with Collaborating/Supervising Physician regularly, educating patients on Transgender Health, HIV disease, primary care issues, and adherence strategies to medications prescribed, assessing patient risk behaviors and educating patients in prevention of HIV transmission, providing HIV prevention therapy as needed, assisting with the development and implementation of clinic policies and procedures, clinical protocols, continuous quality improvement standards and program monitoring, developing strong collaborative relationships with the trans and gender nonconforming community to enhance patient access to services including specialists and consultant care, continually updating own knowledge of trans health and HIV/AIDS through reading, conference attendance, lectures and other educational opportunities, participating in quality efforts to increase health outcomes for patients, demonstrating unconditional positive regard to clients and conducting all aspects of job responsibilities with a focus on exceptional customer service, continuously growing and developing cultural competency, exhibiting an understanding, awareness, and respect for diversity, practicing cultural humility in daily interactions with staff, patients and community, contributing to a positive work environment by demonstrating unconditional positive regard to all Equitas Health employees, interns, etc. with an understanding, awareness, and respect for diversity.
Education/Licensure: Minimum of a Master's Degree in Nursing with board certification as a FNP, ANP, AGNP, or AGPCNP; or Minimum of a Master's Degree in Physician Assistant Studies with board certification as a PA. Active Ohio licensure as a Nurse Practitioner or Physician Assistant with prescriptive authority. Federal DEA license. Certification in CPR/BLS.
Knowledge, Skills, Abilities and Other Qualifications: Experience in Epic preferred. 2 years or more experience in providing primary care desired. 2 years or more experience in providing HIV care desired. Ability to work Saturdays and extended hours on an intermittent and rotating basis. Exemplary customer service as well as written and verbal communication skills. Knowledge of health and social service needs of persons with HIV/AIDS. Must have sensitivity to, interest in and competence in cultural differences, transgender and gender non-conforming patients, HIV/AIDS, minority health, sexual practices, chemical dependency and a demonstrated competence in working with persons of color, and gay/lesbian/bisexual/transgender community. Proficiency in all Microsoft Office applications and other computer applications required. Reliable transportation, driver's license and proof of auto insurance required.
