Clinical Pharmacy Specialist, Internal Medicine $8000 Bonus

Job Summary This position is part of the acute care pharmacy department and functions as the pharmacy expert on the interdisciplinary Internal Medicine team. The IM specialist serves as a leader for the department in their specialty area of practice through organizing and executing department initiatives, participating in quality and practice development, and providing education to pharmacy colleagues, providers, patients, and trainees. Day‑to‑day activities include inpatient rounding on the Internal Medicine service; evaluating, monitoring, and modifying drug therapy to optimize patient outcomes and safety; and precepting pharmacy residents and/or students. Non‑direct patient care activities may include cost‑savings initiatives, committee involvement, and participation in departmental projects and practice advancement efforts. PGY‑1 and PGY‑2 residency from an ASHP‑accredited program, with at least 5 years of progressive clinical practice experience in the specialty (or 8 years progressive practice experience). The PGY‑1 residency must have been completed within the past 5 years. Computer Skills and Certifications BLS (Basic Life Support) certification – course accredited by the American Heart Association (AHA) or American Red Cross (ARC). ACLS (Advanced Cardiovascular Life Support) course accredited by the AHA or ARC required for inpatient positions within 6 months of employment. Eligible for licensure by the Texas State Board of Pharmacy (TSBP) – Texas license within 30 days of hire. Job Duties Perform responsibilities in accordance with organizational and departmental policies, performance improvement objectives, and environmental/infection control standards. Coordinate staff training, competencies, certification, and continuing education specific to the specialty. Assist clinical staff pharmacists with therapeutic monitoring, patient assessments, interpretation of lab values, TPN assessment, and pharmacokinetic calculations. Identify opportunities for process improvements, develop and implement policies and procedures for prompt, safe, and efficient patient care. Promote pharmacy and coordinate multidisciplinary in‑services, meetings, and publications. Manage P&T initiatives, monitor documented interventions, verify data, develop measures, and report outcomes. Support cost‑savings initiatives and assess cost‑effective pharmaceuticals. Review formulary additions, develop monographs for presentation, and manage the formulary system for acquisition of non‑formulary products. Maintain independence and problem‑solving skills, take charge when necessary, and provide direction. Mentor clinical staff pharmacists and support personnel, delegate tasks, evaluate performance, and represent the Pharmacy Department in meetings. Enter computer orders accurately, thoroughly, and timely; review medication profiles and orders for therapeutic appropriateness, duplications, and interactions; document interventions. Participate in educational programs, literature searches, and orientation and training of new employees. Handle core functions such as direct patient interaction, maintaining patient records, and maintaining confidentiality in accordance with policy. Demonstrate professionalism, compliance with safety, infection control, and corporate policies, and effective communication.
