As a Nurse Practitioner (NP) or Physician Assistant (PA) with a background in Geriatrics or Internal Medicine, you will play a crucial role in our mission to reduce hospitalizations and improve patient care standards. About the Role We are seeking a dedicated and experienced Nurse Practitioner (NP) or Physician Assistant (PA) to join our team in State College, PA. In this hybrid role, you will spend time on-site at skilled nursing facilities, conducting patient assessments and developing personalized care plans. You will also work remotely from home, supported by a dedicated scribe team for efficient documentation. Your success will depend on building strong relationships with the facility, maintaining a patient-centered approach, and being a collaborative team player.
