Job Description The Registered Nurse (RN) Lead for the Supportive Crisis Stabilization Center provides basic and essential medical care to clients admitted to the dually certified OMH/OASAS crisis facility. The SCSC is a 24/7/365 program for individuals in Mental Health/Substance Use Disorder (MH/SUD) crisis situations. The SCSC will provide immediate, therapeutic support by certified peers and licensed behavioral health professionals. The RN Lead is part of a multidisciplinary team of staff and will support triage, screening, assessment, interventions, discharge planning and follow-up services for clients. The RN Lead is responsible for the clinical supervision of the other Registered Nurses and/or Licensed Practical Nurses and to ensure clinical documentation is completed for all clients. The RN Lead is responsible for providing quality care to clients utilizing a person-centered approach for all individuals. Duties and Responsibilities Provides nursing support and oversight to RN staff Consults with Program Director when applicable, including accessing the PHMHP on-call support Assist in the daily operations of the certified SCSC supporting individuals with Mental Health/Substance Use Disorder (MH/SUD) challenges Adheres to all state requirements and professional nursing standards as a licensed provider of medical care and treatment Assists with the development of infection‑control policies and protocols, medication administration, storage procedures, and controlled substance requirements Triage, assess, and observe clients to determine appropriate level of care for individuals presenting at the SCSC for admission, using evidence‑based screening tools and a trauma‑informed approach Monitor medical status of clients, including vital signs, basic medical care, examination and observation of signs/symptoms of medical conditions Assist the multidisciplinary team with the development of a Safety Plan and Individualized Service Plan to outline appropriate care, treatment, and recommended services based on client needs Provide therapeutic interventions to clients in crisis including psychoeducation, crisis counseling, and de‑escalation Consult with on‑call Psychiatric Provider for medical/clinical advisement on psychiatric and/or substance use related concerns of clients, as applicable Assist SCSC Program Director with interviewing, hiring, training, scheduling, supervising and evaluating licensed nursing staff Provide clinical oversight and supervision of the licensed nursing staff Maintain accurate records and complete documentation of client services Coordinate the appropriate care and services for clients, which includes: Assist with the development of and ensure the implementation of a Safety Plan and Individual Service Plan outlining care, treatment, and recommended services based on individual client needs at admission and upon discharge Provide information and referrals to treatment facilities or services for SUD Provide information and referrals to clinical/therapeutic services for mental health challenges Provide information and referrals for medical insurance and medical/primary care services Provide information and referrals to services related to social determinant of health needs (housing, financial, food security, transportation, childcare, utilities, safety, etc.) Collaborate with community partners to facilitate appropriate services upon discharge Communicate relevant client information with family members, caregivers, or designated individuals, as appropriate and if authorized Document information related to referrals, coordination of services, and follow‑up contacts and supports provided to client and community providers Ensure OMH and OASAS regulatory requirements for certification are followed Ensure MHAST policies and procedures are implemented and followed Collaborate with other MHAST program staff to provide continuity of care to clients upon discharge Collaborate with community partners and agencies to coordinate services/referrals for clients Help maintain overall cleanliness and safety of the facility; ensure a welcoming and supportive environment for clients Attend agency staff meetings and trainings Cooperate with audits, inspections, investigations Flexibility to potentially cover shifts, in the event of call‑ins or scheduling issues On‑call designation (for medically related issues) Work Allocation RN/Medical Care and Client Services (30%) Coordination of Client Services (30%) Supervision and Leadership (30%) Other (10%) Required Qualifications Bachelor’s degree in nursing with a valid RN License in NYS Three or more years of supervisory experience or leading a team of nurses Four or more years of experience working in MH/SUD field Four or more years of experience working with and/or treating children, youth, and adults Obtain the NYS Peer Specialist Certification within orientation period Valid driver’s license and acceptable driving record to drive agency vehicles Successfully complete all required background and reference checks Complete all state/agency required trainings during orientation period, and annually, as required. Maintain strict confidentiality and professional boundaries Support the mission and vision of MHAST Knowledge, Skills and Abilities Knowledge of MH/SUD issues facing individuals in the community Knowledge and understanding of the characteristics, diagnosis, and treatment options of a mental illness and substance use disorders Ability to handle crisis situations, traumatic events, or sensitive circumstances, support and advocate for individuals in crisis Knowledge of community resources, services, and systems to assist individuals with referrals and links to appropriate care based on individual needs Understanding and sensitivity to support a diverse client population (race, religion, gender identity, sexual orientation, ethnicity, etc.) Trauma Informed Care Training Knowledge of EMR/EHR systems Knowledge of Medicaid/MCO billing Knowledge of OMH and OASAS Regulations Knowledge of Microsoft Office Suite (Excel, Word, Teams, etc.) Excellent communication skills (verbal, written) Working Conditions Crisis center facility, office environment Some exposure to cleaners, chemicals is expected Physical requirements include walking, bending, standing, climbing stairs, and lifting (light/medium‑up to 50 pounds) Travel required when providing transportation to clients, or to travel between MHAST locations May need to work flexible schedule, depending on staffing needs #J-18808-Ljbffr
