Nurse Case Manager - Home & Community Care (RN)

Area Agency On Aging-Western

Western Reserve Area Agency on Aging is seeking a MyCare Ohio Case Manager to manage assessments for long-term clients aged 60 and older. This role requires a current Registered Nurse license and at least one year of experience in home health care or social work.
