Description
  • Conduct comprehensive evaluations of patients in both inpatient and outpatient environments, crafting tailored treatment plans
  • Deliver therapeutic interventions targeting improved mobility, strength, balance, and functional independence
  • Collaborate with a network of healthcare professionals, including nurses, physicians, case managers, and rehab personnel to enhance patient outcomes
  • Educate patients and their families about therapeutic objectives, discharge strategies, and at-home exercises
  • Maintain detailed documentation of evaluations, treatments, and patient progress in compliance with facility and regulatory standards
  • Participate actively in care team rounds and interdisciplinary meetings to ensure seamless patient care
Education
  • Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited institution
Certifications
  • Valid state licensure as a Physical Therapist
  • Preferred CPR certification
  • Additional specialized certifications (e.g., neurological, geriatric) are advantageous, but not essential
Skills
  • Prior experience is highly preferred
