- Conduct comprehensive evaluations of patients in both inpatient and outpatient environments, crafting tailored treatment plans
- Deliver therapeutic interventions targeting improved mobility, strength, balance, and functional independence
- Collaborate with a network of healthcare professionals, including nurses, physicians, case managers, and rehab personnel to enhance patient outcomes
- Educate patients and their families about therapeutic objectives, discharge strategies, and at-home exercises
- Maintain detailed documentation of evaluations, treatments, and patient progress in compliance with facility and regulatory standards
- Participate actively in care team rounds and interdisciplinary meetings to ensure seamless patient care
- Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited institution
- Valid state licensure as a Physical Therapist
- Preferred CPR certification
- Additional specialized certifications (e.g., neurological, geriatric) are advantageous, but not essential
- Prior experience is highly preferred
- Ability to care for patients with intricate medical and mobility challenges
- Strong collaborative and communication skills
- Proficiency in digital charting and clinical documentation systems
