Job Description Job Description Job Summary We are seeking a Social Services Coordinator for a contract/temporary opportunity in Riverside, CA. This role supports veterans by providing comprehensive case management services that promote long-term stability, independence, housing security, employment readiness, and overall well-being. The Social Services Coordinator will work in a mission-driven government and public sector environment where collaboration, community impact, and client advocacy are central to the work. This is an excellent opportunity for a compassionate and organized human services professional who enjoys working directly with clients, coordinating resources, and helping individuals overcome barriers to self-sufficiency. Key Responsibilities

• Conduct comprehensive client assessments to identify needs, barriers, and goals related to housing, employment, education, finances, healthcare, and overall stability.
• Develop, update, and monitor Individual Action Plans while meeting regularly with clients to track progress and adjust service strategies.
• Coordinate services with the VA, healthcare providers, community organizations, public assistance programs, and other supportive service providers.
• Complete intake assessments, verify eligibility, maintain confidential client files, and document all client interactions using DAP case notes.
• Conduct home visits to monitor client safety, well-being, and housing stability.
• Facilitate life skills workshops and educational groups focused on employment readiness, job retention, computer skills, financial literacy, motivation, and overcoming barriers.

Compensation and Benefits

• Pay rate: $26.00 to $28.00 per hour.
