- Conduct evaluations for patients in inpatient and outpatient settings and tailor specific treatment plans.
- Implement physical therapy interventions with an emphasis on mobility, strength, balance, and functional abilities.
- Engage collaboratively with a multidisciplinary team of nurses, physicians, case managers, and rehabilitation specialists to maximize patient outcomes.
- Instruct patients and their families on therapeutic goals, necessary discharge planning, and at-home exercise routines.
- Accurately document evaluations, treatments, and progress following facility and regulatory guidelines.
- Participate in interdisciplinary team rounds and conferences to guarantee coordinated care.
- Doctorate in Physical Therapy (DPT) or its equivalent from an accredited institution.
- State licensure as a Physical Therapist .
- CPR certification preferred.
- Specialized certifications (e.g., neurological, geriatric) are beneficial but not mandatory.
- Prior experience is highly preferred.
- Expertise in managing patients with intricate medical and mobility challenges.
- Proficient communication and teamwork abilities.
- Familiarity with digital charting and clinical documentation systems.
