Description
  • Conduct evaluations for patients in inpatient and outpatient settings and tailor specific treatment plans.
  • Implement physical therapy interventions with an emphasis on mobility, strength, balance, and functional abilities.
  • Engage collaboratively with a multidisciplinary team of nurses, physicians, case managers, and rehabilitation specialists to maximize patient outcomes.
  • Instruct patients and their families on therapeutic goals, necessary discharge planning, and at-home exercise routines.
  • Accurately document evaluations, treatments, and progress following facility and regulatory guidelines.
  • Participate in interdisciplinary team rounds and conferences to guarantee coordinated care.
Education
  • Doctorate in Physical Therapy (DPT) or its equivalent from an accredited institution.
Certifications
  • State licensure as a Physical Therapist .
  • CPR certification preferred.
  • Specialized certifications (e.g., neurological, geriatric) are beneficial but not mandatory.
Skills
  • Prior experience is highly preferred.
