In-Home Registered Nurse - Geriatric Home Care

Pace RI is seeking a Home Care RN to assess participant safety in the community, address acute care needs, reconcile medications, and support safe transitions of care. You will develop initial and ongoing care plans using nursing assessments and attend IDT and Core Team meetings as scheduled. The Home Care RN collaborates with the Team PCP and social worker and works with other team members as needed.
