Utilizes evidence-based practice to anticipate expected patient outcomes, which support the mental health diagnosis.Job Description:Schedule:Monday thru Friday days shift except for one evening a week (typically until 7pm) through, dayKEY ACCOUNTABILITIES:Assesses, diagnoses when necessary, and evaluates patient’s status and the effectiveness of mental health care using tools deemed appropriate by clinical setting, such as the Diagnostic and Statistical Manual of Mental Disorders (DSM-5), to create an individualized plan of care that includes cultural diversity and special needs appropriate to the patient population served. Continually assesses patient’s progress and provides interventions and strategies to support the mental health diagnosis.Prepares and maintains all required treatment records, reports, and other clinical information on time and accurately using current technology in accordance with regulatory requirements and department procedures.Works collaboratively with the patient and others to develop a comprehensive treatment plan that addresses the needs of the patient, desired outcomes, and agreed upon plan of care.Develops treatment plans and objectives in collaboration with patient and family, as appropriate, based on patient needs/goals. Anticipates and intervenes in areas of patient safety and symptom reduction. Is able to work in a team environment, remaining attentive to ideas, recognizing responsibilities, and actively participating with others to accomplish assignments and achieve desired goals.Coordinates referral options and serves as a link between patients and treatment providers.
