We are seeking a skilled and compassionate Registered Nurse (RN) to join our team at Cornell Care & Rehabilitation Center. The successful candidate will possess a strong foundation in nursing principles and practices, with a focus on delivering high-quality patient care in a rehabilitation setting. As a valued member of our team, you will work collaboratively with our multidisciplinary staff to design and implement individualized care plans that promote optimal recovery and well-being.

Responsibilities:

• Provide direct patient care, including administering medications, treatments, and therapies as prescribed by physicians and other healthcare professionals

• Monitor and assess patient conditions, identifying potential issues and implementing interventions as necessary

• Collaborate with physicians, therapists, and other healthcare professionals to develop and implement individualized care plans

• Communicate effectively with patients, families, and interdisciplinary team members to ensure seamless transitions and optimal care

• Maintain accurate and detailed records of patient care, including progress notes and treatment plans

• Participate in quality improvement initiatives and contribute to the development of policies and procedures
