Registered Nurse (Community-Based / Care Management Nurse)

Aging & Community-Based Services

Location : Northern Worcester County, MA

Remote: Full-Time (35 hours/week)

Compensation: $34-$37

What You’ll Do (Day-to-Day Impact)

· Conduct in-home and community-based nursing assessments for older adults

· Evaluate medical, functional, cognitive, and psychosocial needs

· Collaborate with care managers, physicians, and interdisciplinary team members

· Participate in care planning to support safe community living

· Monitor health status and identify changes requiring clinical intervention

· Provide education to consumers and caregivers regarding health conditions and services

· Support transitions of care including hospital and rehabilitation discharges

· Maintain accurate and timely documentation in compliance with program and regulatory requirements

What We’re Looking For

· Active Massachusetts Registered Nurse (RN) license in good standing

· Experience in community health, care management, geriatrics, or home care preferred

· Strong assessment and clinical decision-making skills

· Ability to work collaboratively as part of an interdisciplinary team

· Excellent communication and organizational skills

· Valid driver’s license and reliable transportation (mileage reimbursed)
