Alternative Home Health + Care Inc

Home health agency servicing patients in the Chicagoland area with home nursing and therapy needs. Job Description Job Summary Responsibilities Patient assessment and observation, administration and monitoring of prescribed medication, treatments and therapies in patient's home environment Follow designed Plan of Care Communication with other members of the patients' multi-disciplinary team Prepare clinical notes and update primary Physician Health promotion and teaching Qualifications Graduate of accredited school of nursing; LPN Program Current and unrestricted licensure in IL Valid driver's license and auto insurance Be able to work and travel independently Knowledge of basic computer operations such as typing, Windows OS, internet, and email CPR certificated Excellent observation, verbal, and written skills At least 3 year of clinical patient care experience in hospital or other clinical setting Preferred at least 1 year home health visiting experience Additional Information All your information will be kept confidential according to EEO guidelines. #J-18808-Ljbffr Alternative Home Health + Care Inc
