As a Care Manager RN, you will play a crucial role in coordinating and managing patient care, ensuring the highest quality of service in a home health setting.

In this role, you will be responsible for:

Developing and implementing individualized care plans for patients in a home health setting
Collaborating with interdisciplinary teams to ensure comprehensive patient care
Monitoring patient progress and adjusting care plans as necessary
Providing education and support to patients and their families
Maintaining accurate and timely documentation of patient care

Requirements for this position include:

Registered Nurse (RN) licensure in the state of practice
Cardiopulmonary Resuscitation (CPR) certification or Basic Life Support (BLS) certification from an approved vendor
Valid Driver's License and must be at least 21 years of age or older
Current automobile insurance and a reliable automobile
Need 1 year experience in setting
