As a Care Manager RN, you will play a crucial role in coordinating and managing patient care, ensuring the highest quality of service in a home health setting.

In this role, you will be responsible for:

• Developing and implementing individualized care plans for patients in a home health setting
• Collaborating with interdisciplinary teams to ensure comprehensive patient care
• Monitoring patient progress and adjusting care plans as necessary
• Providing education and support to patients and their families
• Maintaining accurate and timely documentation of patient care

Requirements for this position include:

• Registered Nurse (RN) licensure in the state of practice
• Cardiopulmonary Resuscitation (CPR) certification or Basic Life Support (BLS) certification from an approved vendor
• Valid Driver's License and must be at least 21 years of age or older
• Current automobile insurance and a reliable automobile
• Need 1 year experience in setting
