Registered Nurse (RN)

As an RN with ComForCare, you'll play a vital role in delivering high-quality, personalized care while supporting positive outcomes for our clients and their families.

Working within your state's Nurse Practice Act you will:

• Provide services in accordance with the care plan
• Make the initial evaluation visit and regularly reevaluate the patient's needs
• Initiate the plan of care and necessary revisions
• Coordinate services
• Inform staff of changes in the condition and needs of the client
• Prepare clinical and progress notes as well as summaries for each patient visit
• Understands and adheres to established ComForCare policies and procedures
• Take urgent calls from caregivers, clients and client families when necessary
• Assist with medication management
• Monitor and document vital signs and health status
• Educate clients and families on care plans
• Accurately document care and observations
• Serve as the bridge between non-medical support and skilled nursing care

You'll thrive here if you are/have…

• A Graduate of an approved school of professional nursing
• An active, in-good-standing RN license in the state of practice
• Two (2) to Five (5) years of nursing experience
• Two (2) to Five (5) years of case management experience
• Two (2) to Five (5) years of RN administrative experience
• A genuine passion for working with seniors
• A flexible, positive, and energetic attitude
• Prior RN experience in Assisted Living or Memory Care (preferred)
• A professional image in both appearance and behavior
• Strong written and verbal communication skills
