Vaya Health is seeking an Innovations Care Manager (Innovations CM) to provide proactive intervention and care coordination for members across mental health, substance use, development disabilities, and related health services. The role involves working with care teams to close gaps in care and prevent hospitalizations.
The Innovations CM will conduct comprehensive Health Risk Assessments, participate in interdisciplinary care planning, and assist with transitional care when needed.
