Requirements
- Current state license as a Registered Nurse - Required
- Homecare Homebase Software Experience - Required
- OASIS Experience - Required
- 1-2 years experience as a Registered Nurse in a clinical care setting - Required
- 2+ years Home health experience - Required
- Proof of current CPR certification - Required
- Current driver's license - Required
- Must have reliable transportation and insurance - Required
- CHPN certification - Preferred
- Bachelor of Science in Nursing - Preferred

Roles & Responsibilities
- Assess home health and/or hospice patients to identify needs and develop plans of care.
- Implement, document, and evaluate the Plan of Care to ensure quality and continuity of care.
- Educate patients and families on care, safety, medications, and self-care practices.
- Supervise and provide clinical direction to assistive personnel to ensure quality service.
- Assess and educate patients on pain management techniques.
- Provide effective communication and serve as a liaison between patients, families, and interdisciplinary teams.
